The term ‘die-off’ is sometimes used in relation to the side effects of live cultures. The phenomenon of ‘Jarisch-Herxheimer Reaction‘, which was first identified in the 1950s, is believed to be the cause of these temporary side effects. In this paper, we describe a case of a patient who developed a severe, life-threatening reaction to a live culture of Lactobacillus rhamnosus GG (LGG).
The patient was admitted to the intensive care unit (ICU) with severe abdominal pain, nausea, vomiting, and diarrhoea. The diagnosis was made based on the patient‘s history and physical examination, as well as the results of blood tests, which showed a high level of bile acids (BALB/c) and low levels of bilirubin (BLB), a marker of liver damage.
In addition, there was evidence of sepsis and septic shock, both of which were consistent with the diagnosis of an acute bacterial infection. However, the cause of death was not determined until the end of the ICU stay, when a liver biopsy was performed. After this, it was determined that LGG was the causative agent for the death.
